ProKatchers is seeking a local contract nurse RN Case Management for a local contract nursing job in Philadelphia, Pennsylvania.

Interview Process: 2 rounds – In-person

Must-Have Qualifications:

Active RN license required; Pennsylvania RN license required upon hire.
Minimum 5 years of professional experience.
Prior inpatient hospital case management experience required.
Strong discharge planning and care coordination experience.
Bachelor's degree required per the hiring requirements.
Experience collaborating with physicians, nursing, social work, and interdisciplinary teams.
Ability to independently manage 18–25 patients per day.
