Description
A-Line Staffing Solutions is seeking a local contract nurse RN Case Management for a local contract nursing job in Buffalo, New York.

Registered Nurse (RN) Case Manager – Remote
Pay Rate: $45-46 per hour
Location: New York (can live anywhere in EST but must hold a NY RN license)
Work Arrangement: 100% Work From Home
Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST
Key Responsibilities
- Conduct comprehensive member assessments using clinical judgment and available health data.
- Develop, implement, and monitor individualized care plans.
- Coordinate healthcare services and community resources.
- Manage members with chronic conditions such as diabetes, hypertension, asthma, and other complex health needs.
- Provide telephonic outreach, education, and health coaching.
- Collaborate with physicians, behavioral health providers, and interdisciplinary care teams.
- Identify barriers to care and develop solutions to improve health outcomes.
- Document assessments, interventions, and member interactions in the case management system.
- Ensure compliance with regulatory requirements, clinical guidelines, and company policies.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license in New York.
- Minimum of 3 years of clinical nursing experience in a hospital, home health, ambulatory care, or related healthcare setting.
- Experience working in healthcare or managed care environments.
- Strong assessment, care coordination, and communication skills.
- Proficiency with computer systems and electronic documentation.
- Ability to work independently and manage multiple priorities.
- Must reside in Virginia and be able to travel locally for field visits.
Preferred Qualifications
- Case Management experience.
- Experience managing chronic disease populations.
- Knowledge of community resources and care coordination programs.
Work Environment
- Quiet and dedicated home workspace required.
- Frequent telephone communication and computer-based documentation.
Compensation
- $45-46per hour
- Monday–Friday schedule
- No nights, weekends, holidays, or on-call responsibilities
