Position Overview:
We are seeking an experienced Registered Nurse Care Manager to support care coordination and facilitate high-quality, member-centered care.
The Care Manager will work collaboratively with members, families, physicians, nurses, and other healthcare providers to develop and monitor care plans, coordinate services, identify clinical risks, and support members across the continuum of care.
Required Qualifications:
Active New York State RN License
Bachelor’s degree required
5+ years of clinical experience as a Registered Nurse
Hospital experience required
Care Coordination experience required
Experience reviewing assessments required
Computer literacy required
