HonorVet Technologies is seeking a Nurse Practitioner Home Health for a local contract job in Troy, Michigan.

Position Overview:
- Employment Type: Full-Time, Permanent
- Locations: Multiple locations (based on availability)
- Specialty: Geriatric Primary Care / Home-Based Primary Care
- Patient Population: Geriatric / Older Adults
- Patient Ratio: Average of 5-9 patients a day
- Setting: Home-Based Primary Care
- Base Salary Range: $155k - $165k annually
Schedule:
- Monday–Friday | No weekends or evenings
- No holidays or hospital rounds
- Minimal call responsibilities
Responsibilities:
- Provide personalized, home-based primary care to high-risk geriatric patients, focusing on their individual healthcare needs rather than patient volume.
- Assess, diagnose, and manage chronic conditions while developing individualized treatment plans to improve patient outcomes.
- Coordinate comprehensive care for patients by leveraging experience in primary care, geriatrics, home health, or hospice, ensuring compassionate, patient-centered treatment.
Requirements:
- A licensed Nurse Practitioner or Physician Assistant in the state where the practice is located.
- Federal DEA License/eligibility
- Geriatric training/experience preferred
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver’s license and good driving record
