Registered Nurse (RN) – Long-Term Care / Rehabilitation
We are seeking a compassionate and dedicated Registered Nurse (RN) to provide quality nursing care in a Long-Term Care and Rehabilitation setting.
Job Responsibilities
- Provide direct nursing care to residents in an LTC/Rehab environment.
- Assess residents, monitor vital signs, and recognize changes in condition.
- Administer medications and treatments as ordered.
- Develop and implement individualized resident care plans.
- Monitor residents’ progress and support rehabilitation and restorative care goals.
- Collaborate with physicians, therapists, CNAs, and other interdisciplinary team members.
- Provide wound care, pain management, fall prevention, and infection control as needed.
- Assist residents recovering from illness, injury, surgery, or hospitalization.
- Document assessments, care provided, medications, and resident progress accurately in the EMR.
- Supervise and guide CNAs and other nursing staff.
- Communicate effectively with residents, families, providers, and the healthcare team.
- Promote resident safety, dignity, independence, and quality of life.
Qualifications
- Current and unrestricted RN license in the applicable state.
- Previous LTC, skilled nursing, rehabilitation, or post-acute care experience preferred.
- Strong assessment, communication, documentation, and organizational skills.
- Ability to work effectively in a fast-paced, team-oriented environment.
