Required Qualifications

  • Active and valid Registered Nurse (RN) license.
  • Minimum of 1 year of hospice nursing experience required.
  • Experience with hospice patient assessments, care planning, and case management.
  • Strong clinical assessment, documentation, and care coordination skills.
  • Ability to work independently and collaboratively with a multidisciplinary team.
