ESSENTIAL FUNCTIONS & RESPONSIBILITIES:
- Serve as a care manager, navigator, and advocate for members of the LiveWell program.
- Serve as a champion of healthy aging practices for all program members to improve independence and support members in their goals of living at home as they age.
- Conduct regular health and wellness screenings to help members set and work toward goals for improvement in health and functional ability.
- Coordinate, plan, and oversee member care and services when support is needed at home.
- Collect, track, and analyze member health and wellness data to help members meet their goals and improve levels of independence.
- Coordinate wellness activities, community resources, vendors, and interventions that meet the interests and needs of the membership.
- Collaborate across all Blakeford Senior Life service lines and support program growth and outreach.
- Other duties as assigned
EDUCATION & EXPERIENCE:
- Bachelor's degree in nursing, social work, occupational therapy, or other wellness or rehabilitation disciplines
- 5+ years experience working with older adults in geriatrics, wellness, rehabilitation, care coordination or community-based services
