Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.

 

The Life Sharing Family Support Assistant is a member of the Life Sharing team and plays an important role in supporting medically fragile children and adults supported in Citizen Care’s Life Sharing program.  The Family Support Assistant will report directly to the Life Sharing Coordinator. The Family Support Assistant supports people with every day needs including achievement of personal health and wellness goals, engagement in their communities, promotion of socially valued roles and independence, and exploration of their unique skills to enhance quality of life.

Responsibilities:

Providing Supports, Interventions & Services:

  • Supporting people with typical daily activities including but not limited to bathing, grooming, personal hygiene, eating, and dressing
  • Supporting effective communication between the person supported and others
  • Assessing needs to identify any conditions or behavior patterns that require medical interventions and supports
  • Providing crisis prevention and intervention services

Maintaining Health & Safety of the Individuals:

  • Completing tasks directly related to maintaining and ensuring health including but not limited to taking and recording vital signs, blood pressure, temperature, heart rate, respiration rate
  • Following individualized care plans to best support the person’s overall health and safety
  • Communicating all identified needs with the care team and family
  • Lifting and transferring to assist people with mobility, facilitating motion exercises and physical therapy

Engaging Individuals in Socially Valued Roles

  • Advocating for maximum independence and community inclusion for people supported
  • Empowering people to make their own choices
  • Offering education, training, and self-development for people in accordance with the Individual Support Plan (ISP)
  • Building and maintaining relationships with important people in the person's life (family, friends, supporters)

Documentation & Administrative Requirements

  • Maintaining the content, integrity, and security of all documentation, whether in paper or electronic format, that comprises the health and care record(s) of service and support delivered. All documentation must include person-centered and trauma-informed language.
  • Demonstrating proficiency in the use of technology in documentation (including but not limited to electronic client records software, Microsoft Office, email, payroll, time keeping, etc.). Accurately and thoroughly completes and submits timely, all required documentation in accordance with organization, program, and payer specific, policies, practices, procedures, and guidelines.

Completion of other duties as assigned by the Life Sharing supervisory staff.

Qualifications:
  1. High School Diploma or GED
  2. Nursing Assistant Certification (CNA), preferred
  3. Must be 18 years of age
  4. Reliable means of transportation
  5. Valid Driver’s License, with excellent driving record
  6. The ability to lift and transfer 50 pounds without restrictions
  7. Excellent communication and writing skills
  8. Act 33, 34 and FBI Clearances
  9. Flexible schedule – willing to work varying days and hours based on the person/family’s needs
Minimum Compensation: USD $18.50/Yr. Maximum Compensation: USD $20.00/Yr.

Milestone Centers supports individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties.
