GENERAL DESCRIPTION OF POSITION:

Responsible for performing a variety of duties working collaboratively with all Clinical and Administrative staff. Duties will involve both clinical and clerical to deliver optimal patient care services within the maximum scope of the LVN's professional expertise. Will perform duties to ensure the patient has a pleasant and satisfactory experience in the way their health care is delivered, managed, coordinated with both internal and external entities and in which the patient participates in decision-making. As a member of the clinical team, this position requires total support to the PCMH principles, policies, and procedures. Assists with patient preparation according to the protocols established for providers or required by special circumstances.

When assessing the patient, the age of the patient is considered. May assist with charge duties on occasion. Aids and guidance to clinical staff as needed. LVN will be able to work in another team in absence or need of a provider in other clinic sites as needed.

ESSENTIAL JOB FUNCTIONS: (with or without accommodations)

Actively and accurately records in the medical record the patient information collected to include updates on medications or summaries from the patient pre-visit summary. PCMH 2C, 3C

Actively participates in a multidisciplinary team, huddles to coordinate care, scheduling and follow up along with the provider and front desk clerks to prepare for the patients scheduled, reviews opportunities for same day access, and alerts other disciplines for patients and families with and without special needs. PCMH 1A, 1B, 1G

Performs and monitors staff administering medication and vaccines orally, rectally, internally or by injections and monitor patient for drug reaction.

Attends and completes required in-service trainings such as HEALH EDU, Su Clinica trainings and other web-based courses and educational programs. Responsible for their own safety as well as the safety of others. Submits required documentation in a timely manner (credentialing requirements, license renewals, certifications, CNE attendance, etc.). Attends work on a regular and predictable schedule in accordance with clinic leave policy and perform other duties as assigned.

Prepares instructs patients and collects and processes specimens for laboratory testing and alerts provider of any patient concerns or abnormal results. PCMH 5A.

Completes required electronic documents to coordinate refills, and orders diagnostic studies/exam through internal or external referral process. PCMH 2C, 3D.

Conducts appropriate screening tests (i.e. glucometers, spirometer) and follows up with provider on tests results. PCMH 5A

Engages patient and their care givers in understanding their care in a culturally and linguistically appropriate manner. PCMH 1F

Observes patients for changes in condition and promptly informs providers of any urgent situations needing immediate action.

Participate in quality improvement projects aimed at improving patient care outcomes for patients. PCMH 6A

Participates in providing training/education to SC employees or students when requested.

Performs patient call backs in a timely manner and documents PCMH 1.2

Performs duties within the scope of practice as delegated by provider through the use of approved standing orders and or protocols. PCMH 2C

Performs nurse coordinator duties by arriving at walk in patients, checking availability of provider's schedule. PCMH 1A

Prepares exam rooms, prepares the patients for examination, treatment, and procedures, and assists the provider.

Takes vital signs, medical history and accurately enters information into electronic medical record, and prepares

patients for examinations, procedures, and treatments. PCMH 2B

1.Teaches method of health promotion and disease prevention in accordance with individualized needs. Refers patient to appropriate resources \ referrals, social services, pharmacy, laboratory, etc. PCMH 3C

2.Verifies that patients and family members' discharge and follow up questions have been addressed and appropriate educational materials provided and documented in the electronic medical records. PCMH 3C

  1. Works jointly with providers to maximize disease prevention and reinforce patient follow through with provider's orders and instructions. PCMH 3C

Laboratory Support: 1. Performs patient testing and reporting as assigned and performs laboratory tests as described in the Laboratory Manual. Performs routine waive tests with accuracy and precision under minimal/no supervision. PCMH 5A

2.Maintains adequate stock levels. PCMH 5A

3.Runs controls, Monitors QC Data and performs, documents and assures proficiency tests are submitted. PCMH 5A

4.Keeps all laboratory records on file and assists in laboratory inspection preparedness. PCMH 5A

5.Maintains instruments following manufacturer recommendation. PCMH 5A

6.Documents accurately maintaining action logs. PCMH 5A

POSITION DIRECTLY SUPERVISED: None

KNOWLEDGE, SKILLS, AND ABILITIES: (use of equipment, job related knowledge, language, etc.)

Knowledge of practice level procedures, techniques, and practice of patient care.

Knowledge and skilled in use of computer and computer software (EMR, Microsoft Word, Excel, etc.) programs as

needed and a telephone system.

Knowledge of regulatory requirements, health plan contracts, governmental benefits, and community resources.

Knowledge of SC policies and procedures.

Knowledge of the Chronic Care Model and Patient-Centered Medical Home Model (PCMH).

Skilled in excellent customer service and makes customer service a top priority.

Ability to complete basic/intermediate math computations and medical math conversions.

Ability to examine documents for accuracy and completeness.

Ability to maintain a flexible work schedule, take calls, work evenings and weekends, and float as needed.

Ability to maintain an effective and professional working relationship with the public and co-workers.

Ability to maintain client, office confidentiality and abides by confidentiality expectations and HIPAA guidelines.

Ability to prepare records in accordance with detailed instructions.

Ability to read, write and speak in a clear, accurate and professional manner; includes active listening skills and

understanding medical terminology.

Ability to recognize, evaluate, solve problems, correct errors, and report to supervisors of issues.

Ability to resolve patient service issues and exercise independent judgment .

Ability to speak, understand and write Spanish and English.

Ability to work in a fast-paced environment.

Ability to work with medical equipment to include but not limited to electronic and manual vital signs measurement,

auditory and visual examinations, oxygen and suction apparatus, electrocardiograms, and respiratory exams.

Able to bend, stretch, stoop, stand, sit, and lift 20 lbs.

Able to insert urine catheters into a specific patient population. Supervises new staff in performance of procedure.

Able to show excellent interpersonal skills reflecting clarity, diplomacy and communicates accurately and effectively

With all levels of staff and management.

Ability to assist with the control of department budget.

Ability to communicate concerns to appropriate personnel using tact and diplomacy.

EXPERIENCE DESIRED/CERTIFICATION REQUIRED: High School or GED. Must be a licensed vocational nurse in the State of Texas. Experience in working with the public preferred. Must be computer literate. Must be current with BLS/CPR. Bilingual (English/Spanish) a must.

SPECIAL INSTRUCTIONS: This position requires travel. Employees must provide transportation. If an employee operates a personal motor vehicle in the performance of their official duties, the employee must possess a current valid Texas driver's license for the appropriate type of vehicle and Texas Liability insurance. This position requires that an employee's driving record be verified with law enforcement to ensure compliance with SCF's driving policy.

Due to the nature of SCF Services, it may be necessary for employees to work extended hours or other variations of the usual shift to ensure adequate care to patients and to maintain service to the community.
