Job Summary:


The LVN provides direct nursing care to medically fragile residents in a CLHF setting. Under the supervision of an RN or physician, the LVN ensures high-quality, compassionate care tailored to each patient’s clinical needs.



  • Essential Duties and Responsibilities

    •  Provide direct nursing care in accordance with physician orders, care plans, and facility policies. 
    •  Administer medications, treatments, and tube feedings as prescribed. 
    •  Perform skilled nursing procedures, including wound care, catheter care, tracheostomy care, and suctioning within scope of practice. 
    •  Monitor and assess residents’ physical and emotional status and promptly report significant changes to the RN, physician, and/or DON. 
    •  Document assessments, treatments, medication administration, and resident responses accurately and timely. 
    •  Maintain Medication Administration Records (MARs) and Treatment Administration Records (TARs). 
    •  Assist with admissions, transfers, discharges, and physician appointments. 
    •  Communicate effectively with physicians, pharmacists, families, and interdisciplinary team members. 
    •  Respond appropriately to emergencies and changes in resident condition. 
    •  Supervise and direct Certified Nursing Assistants (CNAs) and other assigned staff. 
    •  Ensure infection prevention and control practices are followed. 
    •  Participate in care planning and interdisciplinary team meetings. 
    •  Maintain confidentiality and protect resident rights in accordance with HIPAA and facility policies. 
    •  Ensure compliance with California Title 22 regulations and CLHF licensing requirements. 
    •  Participate in quality assurance and performance improvement activities. 
  • CLHF-Specific Responsibilities

    •  Provide care to residents with complex medical needs, including: 
      •  Ventilator-dependent residents 
      •  Tracheostomy care 
      •  Enteral feeding (G-tube/PEG tube) 
      •  Neurological and spinal cord injury patients 
      •  Residents requiring respiratory treatments and oxygen therapy 
    •  Monitor respiratory status and report changes immediately. 
    •  Assist with emergency response procedures and resident stabilization until higher-level medical assistance arrives. 
  • Qualifications

    Required

    •  Current California LVN License in good standing. 
    •  Current CPR/BLS certification. 
    •  Knowledge of medication administration and nursing procedures. 
    •  Ability to work independently and make sound clinical judgments. 
    •  Strong communication and documentation skills. 

  • Physical Requirements

    •  Ability to stand, walk, bend, reach, and lift throughout the shift. 
    •  Ability to lift and transfer residents with assistance and appropriate equipment. 
    •  Ability to respond quickly during emergencies.
Shift & Pay:


  • Shifts: 12-hour shifts, the rate is based on competitive compensation, depending on experience.
