    About the Role

    As an LVN, you'll provide clinical oversight and nursing support across our Assisted Living and Memory Care neighborhoods, helping ensure residents receive exceptional, person-centered care while supporting caregivers and maintaining compliance with California RCFE regulations.

    You'll combine your nursing expertise with compassion, leadership, and teamwork to help residents live with dignity, comfort, and the highest possible quality of life.

    What You'll Do

    Resident Care & Clinical Support

    • Assist with resident assessments and individualized service plans
    • Monitor resident health, document changes in condition, and communicate with physicians, families, and the care team
    • Perform nursing services permitted within California RCFE regulations
    • Support medication management systems and ensure accurate documentation
    • Monitor weight, nutrition, skin integrity, mobility, fall risk, pain management, and overall wellness
    • Maintain accurate resident records and regulatory compliance
    • Participate in care conferences and interdisciplinary meetings
    • Assist with pre-admission assessments when needed

    Leadership & Team Development

    • Provide daily clinical guidance and support to caregiving staff
    • Mentor, educate, and coach caregivers to ensure quality resident care
    • Assist with caregiver training and performance feedback
    • Foster a collaborative, positive team environment
    • Model professionalism, compassion, accountability, and resident advocacy

    Compliance & Quality Assurance

    • Help ensure compliance with California Title 22 regulations, OSHA standards, and company policies
    • Promote resident rights, dignity, privacy, and safety
    • Maintain healthcare supply inventory
    • Participate in quality improvement initiatives
    • Attend required meetings, continuing education, and in-service training

    Qualifications

    Required

    • Current California Licensed Vocational Nurse (LVN) license in good standing
    • Strong assessment, clinical, and documentation skills
    • Basic computer proficiency, including Microsoft Office and electronic documentation
    • Current CPR Certification (or ability to maintain)
    • Current Food Handler Card (or ability to obtain)
    • Required continuing education credits
    • Ability to meet all state health clearance requirements

    Preferred

    • Experience in Assisted Living, Memory Care, Skilled Nursing, or Senior Living
    • Experience mentoring, training, or leading caregiving teams
    • Knowledge of California RCFE regulations and person-centered dementia care

    Employment is contingent upon successfully completing:
