Key Responsibilities

Clinical Care

  • Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs

  • Deliver compassionate, resident-centered care focused on comfort, dignity, and safety

Assessment & Monitoring

  • Observe, assess, and document changes in resident condition

  • Communicate concerns promptly to the RN Supervisor and healthcare providers

Care Planning

  • Assist with the development and implementation of individualized care plans

  • Support resident recovery goals and quality-of-life initiatives

Leadership & Team Support

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Collaborate with interdisciplinary team members to ensure continuity of care

Documentation & Compliance

  • Maintain accurate clinical documentation and medical records

  • Ensure compliance with all state, federal, and facility regulations and standards

Qualifications

Current and active California LVN license required

Current BLS/CPR certification required

Previous skilled nursing or post-acute care experience preferred

Strong clinical judgment and communication skills
