You're a Strong Fit if You Have
  • Current and unrestricted LVN/LPN license
  • Graduation from an accredited vocational nursing program
  • One or more years of nursing experience preferred
  •  Experience in chronic care management, telehealth, remote monitoring, home health, ambulatory care, or population health preferred
  •  Familiarity with EHR systems and clinical documentation
  • Knowledge of chronic disease management and HIPAA requirements
  • Strong communication and organizational skills
