Through our Remote Patient Monitoring (RPM) and Remote Therapeutic Monitoring (RTM) programs, we help patients manage chronic conditions, stay on track with treatment plans, and receive support from the comfort of home. You're a Strong Fit if You Have

• Current and unrestricted LVN/LPN license
• Graduation from an accredited vocational nursing program
• One or more years of nursing experience preferred
•  Experience in chronic care management, telehealth, remote monitoring, home health, ambulatory care, or population health preferred
•  Familiarity with EHR systems and clinical documentation
• Knowledge of chronic disease management and HIPAA requirements
• Strong communication and organizational skills
