Licensed Social Workers / Mastered Social Workers

Provide mental or clinical social work services both telephonically and in the home of clients Carry out comprehensive social work assessments by phone or in-person via home visits todevelop a plan of care for the client Work directly with patients and their families to address unmet needs and provide thenecessary social work services to help patients and support achieve their care goals Maintain relationships with referring partners through consistent and timely communication Promote the STARS program and the STARS CRDC Resource Centers program offerings byhelping clients to connect with programs that address their needs Evaluate patients (existing, new, and prospective) based on their needs, limitations, and desires Address concerns and goals while maintaining constant communication with the patient Build rapport with patients and their families Respond to crisis situations that affect vulnerable or mentally unstable seniors Advocate and raise awareness on behalf of clients and the needs of the community. 90-Day Incentive Bonus ranging from $300 to $600- & first-year incentive for maintaining volumes Both In-Person and Telehealth visits with clients – must be willing to conduct some visits in-person.
