We are looking for a Licensed Social Worker to support residents and families at our skilled nursing facility in Ellicott City, MD. This is a full-time, on-site role handling psychosocial assessments, care planning, discharge planning, and resident and family support as part of the interdisciplinary team.
Qualifications:
- Active Maryland social work license (LMSW, LGSW, or LCSW-C)
- Experience in skilled nursing, long-term care, or a healthcare setting preferred
- Knowledge of psychosocial assessment, care planning, and discharge planning
- Strong communication and documentation skills
Responsibilities:
- Complete psychosocial assessments and MDS social work sections
- Develop and update care plans with the interdisciplinary team
- Coordinate discharge planning and community resources
- Support residents and families through admission, stay, and transitions
- Maintain accurate documentation and regulatory compliance
