How you'll make an impact in this role:

• Design Person-Centered Care: Build comprehensive transition plans that bridge the gap between hospital and home, ensuring every step reflects the patient's unique needs and goals.
• Champion Patient Advocacy: Proactively identify and dismantle barriers to care, ensuring your patients have the resources and support systems they deserve.
• Act as a Clinical Subject Matter Expert: Serve as the go-to resource for the care team on complex issues, including crisis intervention, neglect, and adjustment needs.
• Drive Care Excellence: Maintain a continuous, collaborative pulse with supervisors and transition teams to refine health plans and ensure seamless implementation.

What minimum qualifications you'll need:

Licensure / Certification / Registration:

• One or more of the following required:
  
  • Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
  • Licensed Clinical Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
• Preferred Credential(s):
  • Drivers License.

Education:

• Master's degree in Social Work required.

Work Experience:

• 3 years of experience preferred.
