Job Overview
This role primarily conduct diagnostic assessments and referrals for programmatic care. Determines clinical interventions, facilitates therapy, and provides education for patients and their families. Completes Diagnostic assessment and bills under a physician. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary team and all other contacts. Functions as an interdisciplinary team member.
Schedule: 8-4:30, Monday - Friday
Locations: Maplewood and Edina
