    Description

    Job Duties:

    • Assessing Social and Emotional Factors: Evaluate how illness affects patients' social, emotional, and psychological well-being.
    • Direct Social Work and Counseling: Provide support to patients and their families through counseling and social work services.
    • Resource Utilization: Connect patients and families with available resources for social, emotional, and practical support.
    • Practical Assistance: Help with financial or environmental challenges as necessary.
    • Caseload Management: Manage and track the productivity and status of patients.
    • Record Keeping: Maintain accurate patient records, including progress notes and reports.
    • Collaboration: Work with physicians and other healthcare providers to address social and emotional factors affecting health.
    • Consultation: Provide guidance to external agencies or internal departments on social and emotional factors affecting patients.
    • Communication: Ensure accurate communication with both internal and external stakeholders.
    • Professional Development: Participate in workshops and professional meetings to stay updated in the field.

    Specific Job Demands (Physical Requirements):

    • Strength: Light work involving occasional lifting of up to 20 lbs and frequent lifting of up to 10 lbs. It may involve walking and standing, even when the weight is negligible.
    • Reaching: Frequent reaching with arms or hands.
    • Handling and Fingering: Frequent use of hands for seizing, holding, and grasping, as well as detailed tasks that require finger dexterity.
    • Talking and Hearing: Frequent need to speak clearly and exchange information with clients, families, and colleagues, as well as hear and understand communication.
    • Vision: Frequent need for clear vision up close, with the ability to adjust focus and identify colors.

    Minimum Education, Knowledge, Skills, and Abilities:

    • Education: Bachelor’s or Master’s degree in social work from an accredited institution.
    • Experience: At least one year of social work experience, preferably in a medical setting.
    • Licensing and Credentials: Required to be licensed and have access to a valid driver’s license in Pennsylvania, with proof of insurance.
    • BLS Certification: Required for basic life support.

    Working Conditions:
