Licensed Practice Nurse (Nights) - Residential

A health services network in Brooklyn, New York is seeking a new Licensed Practical Nurse (LPN) to support residential health services in an ICF setting and work closely with the clinical team to provide direct care and nursing support.

About the Opportunity:

• Shift: Overnight
• Schedule: Sunday to Wednesday
• Hours: 9pm to 9am
• Setting: Residential Facility

Responsibilities:

• Support the Registered Nurse in coordinating health care services for the assigned residence
• Provide direct nursing care, health instruction, and support for individuals receiving services
• Administer medications and help oversee medication storage, ordering, and related records
• Document client progress, maintain accurate health records, and assist with appointment tracking and follow-up
• Assist with clinical needs such as first aid, infection control, examinations, and transport to appointments

Qualifications:

• At least 1 year of experience as a Licensed Practical Nurse providing community-based health services to individuals with developmental disabilities
• Current New York State Licensed Practical Nurse license and registration
• Graduate of an accredited professional nursing program
• Experience supporting individuals with tracheostomy care, insulin administration, nebulizer treatments, and wound care
• Ability to read, write, speak, and understand English
• Strong oral and written communication, organizational, and interpersonal skills
• Ability to maintain confidentiality and work effectively with coworkers, families, and outside contacts
• Knowledge of commonly used adaptive equipment such as wheelchairs and related devices
• Must be willing to work at various program sites
• OPWDD clearance

Desired Skills:

• Experience in residential or ICF settings
• Familiarity with individuals with intellectual and developmental disabilities
• Experience assisting with AMAP recertification files
• Experience collaborating with interdisciplinary care teams
