About the Role:New Approach Healthcare Inc is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Stoughton, MA. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality home care services to Adult Foster Care and Group Adult Foster Care (AFC/GAFC) patients across diverse settings. Responsibilities:- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation skill.
- Monitor patients' conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns appropriately through agency procedures.
- Assist with activities of daily living such as mobility support, and basic life support during emergencies.
- Collaborate with physicians, case managers, and caregivers to develop and implement individualized care plans aligned with long-term care regulations.
- Maintain meticulous medical documentation in compliance with HIPAA and clinical confidentiality policies.
- Educate patients and their families on health management strategies, including dementia care, Alzheimer's support, and medication administration.
Requirements:- Active LPN license in the state of Massachusetts (required)
- Graduate of an accredited Licensed Practical Nursing program
- Current BLS/CPR certification
- Previous clinical experience in a healthcare setting preferred
- Proficiency with electronic medical records (EMR) systems
- Strong communication, organizational, and critical thinking skills
- Compassionate, patient-focused demeanor with a commitment to quality care
