    Description

    Make a Difference Without Leaving Home

    Prime Medical Services is seeking an experienced, compassionate, and highly organized Licensed Practical Nurse (LPN) to join our growing Chronic Care Management (CCM) team.

    This fully remote role provides an opportunity to make a meaningful impact on the lives of residents living in skilled nursing facilities through proactive care coordination, chronic disease management, and resident engagement. Working closely with Nurse Practitioners, facility nursing teams, residents, families, pharmacies, and specialists, you will help improve outcomes, support continuity of care, and reduce avoidable hospitalizations.

    This position is ideal for experienced long-term care and skilled nursing nurses seeking a remote role focused on resident relationships, care coordination, and improving quality of life without routine bedside assignments.

    What You'll Do

    • Conduct monthly Chronic Care Management encounters with residents using telephone and approved telehealth platforms.
    • Review resident census information and prioritize residents who have experienced hospitalizations, falls, medication changes, weight loss, or other significant clinical events.
    • Monitor residents with chronic conditions and identify changes that may require additional follow-up.
    • Coordinate care with facility staff, primary care providers, specialists, pharmacies, hospitals, and family members.
    • Complete post-discharge and care transition follow-up activities.
    • Identify and escalate urgent clinical concerns to the supervising Nurse Practitioner or RN Director.
    • Support completion of monthly resident engagement activities and documentation required for CMS Chronic Care Management services.
    • Maintain accurate and timely documentation within electronic health record systems.
    • Participate in daily team huddles and interdisciplinary communication.
    • Assist with quality assurance initiatives, compliance monitoring, and CCM program improvement efforts.
    • Help ensure services are documented in accordance with CMS and organizational requirements.

    Qualifications

    Required

    • Must be an Illinois resident.
    • Active, unrestricted Licensed Practical Nurse (LPN) license in at least one U.S. state.
    • Ability and willingness to obtain and maintain Illinois LPN licensure.
    • Minimum of three years of nursing experience.
    • Experience in skilled nursing, long-term care, care coordination, chronic disease management, case management, population health, or telehealth nursing.
    • Strong communication, organization, and resident engagement skills.
    • Ability to recognize significant changes in condition and appropriately escalate concerns.
    • Experience with electronic health records and clinical documentation systems.
    • Ability to work independently while collaborating effectively with interdisciplinary teams.
    • Knowledge of HIPAA requirements and resident confidentiality standards.
    • Reliable high-speed internet connection and dedicated home office workspace.

    Preferred

    • Skilled Nursing Facility (SNF) experience.
    • Chronic Care Management (CCM) experience.
    • Medicare population experience.
    • Transitional Care Management or care coordination program experience.
    • Familiarity with CMS Chronic Care Management guidelines and documentation requirements.
