This is a PRN (as-needed) position, with shifts assigned based on patient volume, staffing needs, operational requirements, and available shifts, and does not guarantee a minimum number of hours or shifts. The employee must maintain availability, report for accepted and scheduled shifts, provide timely notice of absences in accordance with FHC policies, and may be required to work at various FHC service locations based on organizational needs. The employee must complete required orientation, training, competency assessments, and annual education and maintain current CPR/BLS certification, South Carolina LPN licensure, and all other required credentials.
ESSENTIAL JOB DUTIES
- Assesses and monitors patient conditions and conducts health risk assessments within the LPN scope of practice.
- Administers and reconciles medications, treatments, and immunizations as prescribed; maintains accurate medication lists and documents OTC medications, supplements, and alternative therapies.
- Provides medication information, health education, and health behavior counseling as directed by the provider.
- Prepares patients for examinations, treatments, diagnostic procedures, and other clinical services.
- Obtains and processes laboratory specimens and performs authorized in-house testing according to established procedures.
- Notifies patients of laboratory and radiology results as directed by the provider and tracks results requiring follow-up.
- Recognizes changes in patient condition and communicates concerns promptly to the provider, RN, or appropriate clinical team member.
- Coordinates assigned patient panels, identifies gaps in care, and assists with patient follow-up and care coordination.
- Makes and tracks referrals, including referrals to community resources, as directed by the provider.
- Maintains accurate, timely, complete, and confidential patient medical records and clinical documentation.
- Participates in pre-visit planning, daily team huddles, and team-based care activities when scheduled.
- Maintains quality control of in-house waived testing equipment, monitors refrigerator temperatures, and completes required documentation.
- Sterilizes and maintains equipment according to written protocols and orders and stocks clinical supplies as assigned.
- Follows all infection prevention, safety, biohazardous waste disposal, exposure control, and corporate compliance policies and procedures.
- Provides respectful, courteous, and responsive service to patients, visitors, providers, and co-workers and performs other duties as assigned or delegated.
- Graduate of an accredited Licensed Practical Nursing program.
- Current, unrestricted South Carolina LPN license required.
- Current CPR/BLS certification required.
- Two years of nursing experience in a healthcare setting preferred.
- Primary care, ambulatory care, community health center, or FQHC experience preferred.
- Experience with electronic medical records/EHR systems preferred.
- Must meet FHC credentialing, competency, background screening, and employment requirements.
- Must maintain all required licenses, certifications, and credentials throughout employment.
The Licensed Practical Nurse (LPN) provides safe, compassionate, and patient-centered nursing care in accordance with the LPN scope of practice, provider orders, FHC policies and procedures, and applicable federal and state requirements. The PRN LPN supports the clinical team on an as-needed basis based on patient volume, staffing needs, and operational requirements and works collaboratively with providers and other members of the healthcare team to promote quality care and positive patient outcomes.
