Position Overview:
• We are seeking a dedicated Licensed Practical Nurse (LPN) to support an outpatient primary care team in the Bronx.
• The LPN will work closely with providers and other clinical staff to provide patient care, assist with clinical procedures, obtain vital signs, administer medications as directed, and support efficient day-to-day operations of the health center.
Responsibilities:
• Obtain and document patient vital signs and medical histories.
• Prepare patients for examinations and procedures.
• Assist physicians, NPs, and PAs with patient care and clinical procedures.
• Administer medications, injections, and treatments as ordered and within scope of practice.
• Perform routine nursing tasks and follow established clinical protocols.
• Provide patients with basic health education and instructions.
• Assist with preventive care and chronic disease management activities.
• Document patient information accurately in the electronic medical record.
• Assist with follow-up calls, referrals, and coordination of patient care.
• Communicate patient needs and clinical information to the appropriate members of the care team.
• Maintain a clean, safe, and organized clinical environment.
• Provide respectful, culturally sensitive care to a diverse patient population.
Qualifications:
• Active New York State LPN license.
• Graduate of an accredited practical nursing program.
• Current BLS certification.
• Prior outpatient, primary care, community health, or FQHC experience preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary healthcare team.
• Bilingual skills are a plus.
