Description
A health services network in New York City is seeking a Licensed Practical Nurse (LPN) to join their community-based behavioral health program in Brooklyn.
About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: 9am to 5pm (17.5 hours per week)
- Setting: Community-based / Flexible
Responsibilities:
- Provide medication administration and support, including oral and intramuscular medications
- Monitor medication adherence, effectiveness, and potential side effects
- Perform phlebotomy and assist with coordination of healthcare services
- Deliver health education and psychoeducation related to medications, wellness, and recovery
- Assess clients' mental status and healthcare needs, and complete required clinical documentation
Qualifications:
- At least 3 years of experience working with adults ages 18+ in psychiatric or behavioral health nursing, substance use treatment, social service settings, homelessness services, justice-involved populations, or related support settings
Graduate of a Practical Nursing program
- Current and unrestricted New York State Licensed Practical Nurse license
- Knowledge of community-based treatment, support services, and resources
- Strong organizational skills with the ability to work independently and self-motivate
- Proficiency with computers and electronic documentation
Desired Skills:
Bilingual (English and Spanish)
