Position Overview
LPN and Medicare Annual Wellness Visit Nurse:
Promotes and restores patients' health by completing the nursing process; collaborating with physicians and
multidisciplinary team members; providing physical and psychological support to patients, friends, and families;
supervising assigned team members.
Working in close collaboration with other departments and programs, the Medicare Annual Wellness Visit Nurse also builds individual and community capacity by increasing the knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

NO WEEKENDS
Compensation
Position starts at $20.00 (LPN) however, PHN recognizes experience, education and site location when determining wage.
Qualifications
Graduate of an approved school as an LPN - and two years' experience in a clinical setting
Infection Control and strong clinical knowledge
Administering Medication
Previous venipuncture and injection experience
Previous experience in calling prescriptions under a provider's direction
Computer proficient
