Position Overview
We are seeking a compassionate, dependable, and detail-oriented Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality nursing care under the direction and supervision of a registered nurse (RN), physician, or other authorized healthcare provider.

Key Responsibilities
Patient Care
- Provide direct nursing care to patients in accordance with their individualized care plans.
- Monitor and document patients' health status, vital signs, symptoms, and responses to treatment.
- Administer medications and treatments as ordered and within the scope of practice.
- Assist with wound care, dressing changes, and other nursing procedures.
- Collect and prepare laboratory specimens as directed.
- Assist healthcare providers with examinations, treatments, and procedures.
- Recognize changes in patients' conditions and promptly report concerns to the appropriate healthcare provider.
- Assist patients with activities of daily living as needed.
- Promote patient comfort, safety, dignity, and independence.
- Provide patient and family education within the LPN's scope of practice.
Medication & Treatment Management
- Administer medications safely and accurately according to provider orders and facility policies.
- Monitor patients for medication effectiveness, adverse reactions, and potential complications.
- Document medication administration and treatments accurately.
- Maintain appropriate medication storage, security, and inventory procedures.
- Follow established infection-control and medication-safety protocols.
Documentation & Care Coordination
- Maintain accurate, timely, and complete patient records.
- Document assessments, interventions, medications, treatments, and patient responses in the electronic health record (EHR) or applicable system.
- Communicate patient updates and concerns to RNs, physicians, and other members of the healthcare team.
- Participate in care planning and interdisciplinary team meetings as appropriate.
- Assist with referrals, follow-up appointments, and coordination of patient services.
Patient & Family Support
- Establish professional and compassionate relationships with patients and families.
- Answer routine questions and provide appropriate health education.
- Support patients and families in understanding treatment plans and care instructions.
- Maintain patient privacy and confidentiality in accordance with HIPAA and applicable regulations.
- Advocate for patient needs and communicate concerns to the appropriate healthcare professional.
Safety & Compliance
- Follow applicable federal, state, and local healthcare regulations.
- Maintain compliance with HIPAA, OSHA, infection-control standards, and organizational policies.
- Follow emergency and patient-safety procedures.
- Maintain a clean, safe, and organized clinical environment.
- Participate in quality-improvement and patient-safety initiatives.
Qualifications
- Graduate of an approved Practical Nursing program.

Current, unrestricted Licensed Practical Nurse (LPN) license in the applicable state.
- Current CPR/BLS certification, as required.
- Previous LPN experience preferred; new graduates may be considered based on position requirements.
- Knowledge of nursing principles, medication administration, patient care, and clinical procedures.
- Experience with electronic health records preferred.
- Strong communication and interpersonal skills.
- Excellent organization and attention to detail.
- Ability to work effectively as part of a multidisciplinary healthcare team.
