    Job Description:

    Schedule: Full-Time, 40 hours/week. Monday through Friday, 8:00am to 4:30pm.

    Summary:

    • LPN who provides unit-based staff support focused on implementation of IBPs and transfer of skills. The LPN models effective communication and care approaches, reinforces routines, assists with behavior tracking, and supports consistency across shifts under the direction of the G-BSC and unit leadership.

    Responsibilities:

    • Gathers relevant patient information pertinent for the nursing assessment and assists the BS-C in developing and modifying the plan of care.
    • Model and reinforce IBP strategies during routine care (ADLs, toileting, bathing, meals, transitions).
    • Use agreed-upon scripts and de-escalation steps
    • support early identification of triggers and warning signs.
    • Support collection of objective behavior data (frequency, duration, antecedents, consequences) using approved tools.
    • Coach peers informally at the bedside (with G-BSC guidance) and help orient inexperienced staff to resident-specific IBP strategies
    • Participate in huddles/case conferences as assigned
    • relay frontline observations that inform plan updates.
    • SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:
    • Maintain resident dignity, privacy, and safety
    • follow all NCF policies and regulatory standards.
    • Assists the BS-C in implementing patient/significant others\' teaching plans.

    Credentials:

    • Licensed Practical Nurse (Required)
    • Current licensure as a Licensed Practical Nurse, issued by a program for practical nursing accredited by the PA State Board of Nursing.
    • Basic Cardiac Life Support (Required)
    • CPR Certification required within 30 days of hire.

    Education or Equivalent Experience:

    • H.S. Diploma/GED (Required)
    • Education Specialization: Equivalent Experience: •1+ years One (1) year of hospital/clinical experience
