The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician.Job Description
The LPNdemonstratesknowledge of nursing principles for the resident in the relevant clinical setting. Demonstrates competency and practices within the full scope of nursing expertise/knowledge andutilizesappropriate ageand population specific standards related to the physical and psychosocial needs of the resident as per care plan. The LPNpossessesthe ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. The LPNdemonstratesthe abilityto recognize problems, recommendsolutions,and respond effectively toassistin anemergentsituation.The LPN ensures the health,comfort,and safety of residents. Contributes to the assessment of residents and administers medications or treatments as ordered. The LPN provides technical support to healthcare professionals as needed.Documentsresident interactions as needed and the outcomes related to resident care, medication, behavior, activity, and dietary. The LPN skilled inestablishingand maintaining effective working relationships withresidents, health care providers, and the public. Ability to work effectively with professional and supportive personnel as a member of an interdisciplinary care team. The LPN ability to communicate clearly andmaintainconfidentiality of sensitive information. The LPNcounsels'residents, familymembersand/or resident advocate, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans. The LPN knowledge of resident education principles to develop a self-care program. Performs other related duties as assigned.
Qualifications
