    Position Purpose


    As a Revisit Licensed Practical Nurse (LPN) with Trinity Health At Home, you will deliver compassionate, skilled nursing care to clients in the comfort of their homes. Guided by the physician‑prescribed plan of care and under the supervision of a registered nurse, you will support clients in achieving their highest level of health, safety, and independence. This role is ideal for a mission‑driven LPN who values autonomy, one‑on‑one patient relationships, and the opportunity to make a meaningful difference every day.

    What You Will Do

    • Provide skilled nursing treatments and administer medications as outlined in the medical and nursing care plans.

    • Plan and organize home visits based on client needs, capabilities, physician orders, and available resources.

    • Use interview, observation, and nursing judgment to assess clients, reporting condition changes promptly to the Case Manager and/or physician.

    • Educate family members on client care techniques to support safety and wellness.

    • Participate in interdisciplinary care conferences and contribute to ongoing care plan updates.

    • Apply preventive, therapeutic, and rehabilitative nursing procedures in accordance with the LPN Practice Act and agency standards.

    • Document all care, observations, and evaluations accurately and submit documentation timely.

    • Maintain strong communication with clients, families, and the care team.

    • Adhere to all federal, state, and local regulations, as well as Trinity Health’s Integrity Program and Code of Ethics.

    • Demonstrate commitment to quality improvement and client‑centered outcomes.

    Minimum Qualifications

    • Graduate of an accredited or state‑approved practical nursing program.

    • Current LPN licensure in the state of practice.

    • At least 1 year of acute care experience required; homecare experience preferred.

    • Successful completion of NAPNES or equivalent medication exam if administering medications.

    • Strong interpersonal and communication skills.

    • Reliable transportation and a valid driver’s license.
