Licensed Practical Nurse (LPN) - IDD Group Home

A healthcare organization in New York City is seeking a Licensed Practical Nurse (LPN) to join their team in a residential group home setting in Manhattan.

About the Opportunity:

• Shift: Day shift
• Schedule: Monday through Friday
• Hours: 10:00 AM to 6:00 PM
• Setting: OPWDD residential / group home

Responsibilities:

• Provide medical oversight and support to residential program participants
• Assist with assessment, planning, and implementation of medical care
• Administer and monitor medications in accordance with physician orders and agency policies
• Train and support direct care staff on medical protocols and procedures
• Coordinate medical appointments, follow-up care, and ongoing healthcare needs
• Maintain accurate clinical documentation and communicate with interdisciplinary teams
• Participate in team meetings and provide medically related in-services to staff
• Ensure compliance with OPWDD, agency, and regulatory requirements

Qualifications:

• Active NYS Licensed Practical Nurse (LPN)
• Strong communication, organization, and documentation skills

Desired Skills:

• Experience working with individuals with Intellectual and Developmental Disabilities
• Medical-Surgical experience
• Experience in Residential, Group Home, OPWDD, Behavioral Health, or Long-Term Care settings
• OPWDD experience
• Medication Administration and Staff Training experience
• Care Coordination and Case Management experience
