Their responsibilities include providing skilled nursing care, educating patients, and coordinating care to ensure optimal health and well-being. The LPN's role is vital in promoting comfort and safety and enhancing the quality of life for clients.

    Essential Duties and Responsibilities

    • Deliver skilled nursing services as prescribed by the client’s healthcare provider within the LPN scope of practice.
    • Accurately document all activities, assessments, nursing actions, responses, and care coordination.
    • Educate clients and families on health conditions, self-care techniques, preventative measures, and address nursing needs.
    • Comply with all documentation requirements per company policy, and state and federal regulations.
    • Facilitate clear and effective communication with relevant parties (e.g., physicians, family members, office staff).
    • Coordinate and acquire necessary client supplies, such as durable medical equipment (DME), wound care supplies, and medications.
    • Protect client confidentiality and adhere to HIPAA standards.
    • Provide nursing services including IV management, pain management, and diabetes management for in-home care clients.

    Skills and Qualifications

    • Licensed Practical Nurse (LPN) licensed in the state of Oregon.
    • Minimum of 1 year experience in a healthcare setting.
    • Current CPR/BLS certification.
    • Ability to work independently with minimal supervision; dependable and punctual.
    • Basic proficiency in computer use.
    • Current driver’s license, vehicle insurance, and access to a reliable vehicle or public transportation.
