    As supervised and delegated by a registered nurse, provides direct patient care to patients in accordance with

    applicable scope and standards of practice.

    How you'll contribute

    Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN

    Essential Functions

    1. Makes and documents nursing observations.

    2. Provides input into development and maintenance of the nursing care plan.

    3. Implements assigned aspects of nursing care plan.

    4. Collaborates with RNs and Providers.

    5. Coordinates patient care, admissions, transfers, and discharges with patient care, service and business office personnel, as requested by RNs.

    6. Administers medications, treatment and therapies per scope of practice.

    7. Communicates with patients.

    8. Provides patient and/or family with instruction and education relevant to their condition, as delegated and supervised by RN.

    9. Set up, utilizes and/or maintains equipment, supplies and work area.

    Non-Essential Functions

    1. Assists in the orientation of new employees

    2. Participates in performance/process improvement activities.

    What we're looking for

    Minimum Education

    High school diploma or equivalent required

    Required Skills

    Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

    Must be able to work in a stressful environment and take appropriate action.

    Certifications

    Basic Life Support (BLS)

    Licenses: Current LPN license in state of NC

    Minimum Work Experience

    1 year Electronic Medical Records experience

    1 year Patient Centered Medical Home (PCMH) experience

    1 year quality review/ quality management experience

    3 years ambulatory care experience (Preferred)

    Active North Carolina LPN license (single-state) or compact required. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins
