Licensed Practical Nurse (LPN)

Buckeye Home Health Care of Cincinnati is looking for people who have the heart and skills to work with home bound patients. A strong need for a home health experienced LPN.

Area is determined based on candidate's location

LPN's provide direct patient care under Buckeye Home Health Care

· Provide care wherever the patient calls home.

· Work with the physician for medication needs and changes to the plan of care.

· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.

· When appropriate teach and counsel family members about care and processes for disease/injuries.

· Attend meetings, CEUs, in the Blue Ash office and occasionally in the Dayton office/area.

· Have strong knowledge of skills like lab draws, wound care, medication management, catheter care, etc.
