Position Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team at Crown Heights Center for Nursing and Rehabilitation. As an LPN, you will play a critical role in providing high-quality care to our residents, supporting their medical and emotional needs, and ensuring their comfort and well-being. You will work closely with our healthcare team to promote health, healing, and rehabilitation.
Key Responsibilities
Conduct rounds to observe residents and assess nursing care
Review and implement Nursing Care Plans to address residents’ physical, emotional, and rehabilitative needs
Administer and transcribe physician orders accurately
Collaborate with staff to deliver comprehensive, high-quality care
Manage medication ordering, administration, and documentation
Ensure accurate and thorough documentation of clinical records
Participate in interdisciplinary meetings to improve care and resolve issues
Provide direct patient care as needed, including monitoring vital signs and administering treatments
Assist with identifying and addressing care improvement opportunities
Perform additional duties within the LPN scope of practice as necessary
Qualifications
Valid New York State LPN license
Strong communication, customer service, and interpersonal skills
COVID-19 vaccination required
Preferred: Experience in long-term care and familiarity with PointClickCare
Strong team player with a compassionate, patient-centered approach to care
