Compensation
- Pay Rate:$32–$34/hour
- Weekly Pay:$1,200–$1,274/week(Based on experience)
Schedule
- Shift: Evenings/Nights
- Hours: 5 x 8-hour shifts per week
- Weekend Requirement: Every other weekend
- Shift Assignment: Cannot be changed once accepted
- Start Dates: Available every two weeks based on orientation schedule
Position Overview
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN/LVN) to provide exceptional nursing care in a Long-Term Care (LTC) setting. The ideal candidate will have experience caring for geriatric residents, including individuals with Alzheimer's disease and dementia, while delivering safe, patient-centered care in a collaborative healthcare environment.
Key Responsibilities
- Administer medications and treatments as prescribed by healthcare providers.
- Perform medication passes accurately while following medication safety protocols.
- Monitor residents for changes in condition and promptly communicate findings to the healthcare team.
- Provide direct nursing care in accordance with individualized care plans and facility policies.
- Complete accurate and timely clinical documentation in the electronic medical record.
- Supervise and support Certified Nursing Assistants (CNAs) on assigned units.
- Deliver compassionate care to residents with Alzheimer's disease, dementia, and other chronic health conditions.
- Maintain infection prevention, patient safety, and quality assurance standards.
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members to ensure optimal resident outcomes.
- Perform additional duties within the scope of LPN/LVN practice.
Required Qualifications
Education
- Graduate of an accredited Licensed Practical Nursing (LPN/LVN) program.
Experience
- Minimum 1 year of recent Long-Term Care (LTC) experience required.
- Candidates with recent rehabilitation, medical-surgical, or hospital experience will also be considered.
- Experience caring for residents with Alzheimer's disease and dementia preferred.
Required Licensure & Certifications
- Active New York State LPN/LVN License.
- Proof of active New York license required at the time of submission.
- Basic Life Support (BLS) certification through the American Heart Association (AHA).

Additional Information
- 16-week travel contract assignment.
- Evening/Night shifts with 5 x 8-hour schedule.

- Patient ratio: Approximately 1:18–20.
