  • Assist with daily living activities, including bathing, dressing, and grooming
  • Educate patients and their families about health management and home care
  • Maintain accurate and timely patient records and documentation

We are excited to find a caring, skilled LPN (Licensed Practical Nurse) to join our dedicated home health team.

Responsibilities
• Provide direct patient care under the supervision of a registered nurse.
• Administer medications and treatments as prescribed by physicians.
• Monitor and record patients' vital signs and health status.
• Assist with daily living activities, including bathing, dressing, and grooming.
• Educate patients and their families about health management and home care.
