    Duties and Responsibilities

    Essential Functions:

    • Participates in the development of a written plan of care in accordance with the patient's diagnosis, patient and family needs, physician's orders, and the corporate policies and procedures.
    • Provides selected nursing procedures and treatments to patients as outlined in the plan of care and assigned by the primary nurse.
    • Notifies physician and other care agencies of changes in patient's condition and care needs and obtains new orders, as needed.
    • Assists with coordination of patient care through dialogue with other disciplines involved with the patient and documents same.
    • Participates in patient care conferences.
    • Documents observations and intervention in the patient's clinical record in a timely, legible, and accurate manner.
    • Functions as a member of an interdisciplinary team, encouraging and maintaining open communications at all times.
    • Fosters a climate of mutual respect and teamwork among co-workers and management.
    • Assist in the orientation of new staff.
    • Submits patient visit schedule to Scheduler and communicates significant changes to Primary Nurse.
    • Functions in a cost-effective manner in regard to utilization of time, mileage, supplies, etc.
    • Meets or exceeds established productivity and quality assurance standards.
    • Seeks consultation, guidance and direction from professional or administrative staff, as needed.
    • Informs manager of perceived problem areas related to patient care, corporate management, or community relations.
    • Keeps clinical skills current with nursing practice norms.
    • Exhibits a comprehensive knowledge of, commitment to and demonstrated ability in the practice of home health nursing.
    • Exercises sound judgment and works effectively under pressure.
    • Demonstrates a working knowledge of skills and services provided by each member of the multi-disciplinary team and the ability to relate these to identified patient needs.
    • Acquires and utilizes knowledge of community resources.
    • Elicits feedback from appropriate sources regarding the quality of services provided.
    • Demonstrates a professional attitude, sound judgment and continuity of interactions with physicians, other health care providers and community agencies.
    • Represents the VNA at community activities, as delegated.
    • Promotes a positive corporate image.
