Licensed Practical Nurse - Contract

Schedule: Mon–Fri, 8:00 AM–4:30 PM | 40 hrs/week | No weekends. Location: RGMG Care Managers – Riedman Administrative Campus.

Schedule: Day Shift 5x8-Hour (08:00 - 16:30)

• Schedule: Mon–Fri, 8:00 AM–4:30 PM | 40 hrs/week | No weekends
• Specialty: Adult/Geriatric Care Coordination
• Experience: 1+ year; Home Health/Mobile Case Management
• Cert: BLS required
• Locals: Accepted; lower rate if <75 miles
• First-time Travelers: Accepted
• Floating: Rochester General Hospital, Unity Hospital, patient homes & provider offices
• Travel: Reliable transportation required; home/hospital/office visits may be required
• Role: Transitions of Care / Care Coordination; NO hands-on nursing

Responsibilities:

• Work with RN Care Manager to implement POC
• Telephonic/remote patient screening, follow-up & triage
• Schedule appointments and address barriers
• Patient education & medication adherence
• Biometric monitoring and documentation in EHR
• Home/hospital/provider-office visits as needed
• Connect patients with community resources
• Participate in virtual multidisciplinary care-team rounds
• Manage ~25–30 active patients; 30–40 rotating patients/month
• 30-day program; reassess goals and close/transition to long-term management

Preferred: CHF/COPD & complex-comorbidity experience, community resource knowledge, strong communication/documentation skills.

Key: Primarily Care Coordination/Case Management, not bedside nursing. LPNs do not transport patients.
