Licensed Practical Nurse (LPN) – Rehabilitation & Extended Care

Location: Oneida, NY
Shift: Nights | 7:00 PM – 7:30 AM
Schedule: Every Other Weekend | Holiday coverage per unit needs

Job Summary

Client  is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to provide quality care to residents in a Rehabilitation and Extended Care Facility.

The LPN will work closely with the Unit Manager, Registered Nurses, Physicians, CNAs, and Ward Clerks to ensure residents receive safe, compassionate, and timely care. The ideal candidate will demonstrate strong nursing judgment, communication skills, teamwork, and a customer-service-focused approach.

Key Responsibilities

  • Provide compassionate, resident-focused care with a strong customer service attitude.
  • Collaborate with the interdisciplinary team, including Physicians, RNs, Unit Managers, CNAs, and Ward Clerks.
  • Monitor residents and use sound nursing judgment to identify changes in condition.
  • Communicate changes in resident status promptly to RNs and Physicians.
  • Delegate appropriate assignments and monitor support staff to ensure care is delivered according to treatment plans.
  • Order medications and supplies, prepare equipment, and assist with wound care as needed.
  • Maintain a clean, safe, and healthy environment for residents.
  • Schedule resident appointments and coordinate communication with families regarding appointments and transportation.
  • Enter orders into the computer system, communicate with the pharmacy, and maintain accurate documentation.
  • Participate in meetings, care conferences, and discharge planning as directed.
  • Follow all policies and procedures related to controlled substances, medication administration, and documentation.
  • Promptly report building emergencies, safety concerns, incidents, and suspected abuse, neglect, mistreatment, or exploitation.

Qualifications

  • High School Diploma or equivalent.
  • Completion of an approved Licensed Practical Nurse (LPN) program.
  • Active New York LPN License required.
  • BLS/CPR preferred.
  • Previous experience in rehabilitation, long-term care, skilled nursing, or extended care is preferred.

  • Ability to stand, walk, and sit for extended periods.
  • Ability to kneel and perform repetitive bending movements.
  • Ability to operate a motor vehicle when required.
