What Youll Do
As a Licensed Practical Nurse, youll serve as a medical advocate and trusted healthcare partner for individuals receiving residential supports. Your responsibilities will include:
Provide direct nursing care and monitor the overall health of assigned individuals.
Coordinate medical services and communicate healthcare updates with physicians, nursing leadership, residential staff, and interdisciplinary team members.
Accompany and transport individuals to medical appointments when needed.
Administer and monitor medications and treatments while observing for side effects or changes in condition.
Maintain accurate documentation in Electronic Medical Records (EMR) and other required systems.
Review Individual Service Plans (ISPs) to support medication, behavioral, hygiene, and therapy goals.
Maintain health records in compliance with Department of Developmental Services (DDS) regulations.
Educate and train direct support staff on medication administration and healthcare procedures.
Participate in team meetings, clinical discussions, and required trainings.
Promote a safe, respectful, and person-centered environment for all individuals receiving services.
Participate in an on-call rotation as assigned.

High school diploma/GED/HiSET required
Standard driver's license - class C or class D
Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current MA license.
1 - 3 years of related experience
One year working with individuals with developmental disabilities.
