What Youll Do

As a Licensed Practical Nurse, youll serve as a medical advocate and trusted healthcare partner for individuals receiving residential supports. Your responsibilities will include:

  • Provide direct nursing care and monitor the overall health of assigned individuals.

  • Coordinate medical services and communicate healthcare updates with physicians, nursing leadership, residential staff, and interdisciplinary team members.

  • Accompany and transport individuals to medical appointments when needed.

  • Administer and monitor medications and treatments while observing for side effects or changes in condition.

  • Maintain accurate documentation in Electronic Medical Records (EMR) and other required systems.

  • Review Individual Service Plans (ISPs) to support medication, behavioral, hygiene, and therapy goals.

  • Maintain health records in compliance with Department of Developmental Services (DDS) regulations.

  • Educate and train direct support staff on medication administration and healthcare procedures.

  • Participate in team meetings, clinical discussions, and required trainings.

  • Promote a safe, respectful, and person-centered environment for all individuals receiving services.

  • Participate in an on-call rotation as assigned.

  • High school diploma/GED/HiSET required

  • Standard driver's license - class C or class D

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current MA license.

  • 1 - 3 years of related experience

  • One year working with individuals with developmental disabilities.
