Responsibilities:

• Provide direct patient care including bathing, grooming, dressing, and personal hygiene assistance
• Monitor and record vital signs, reporting changes in patient condition to nursing staff
• Assist patients with mobility, transfers, and ambulation using proper body mechanics and safety protocols
• Support patients with meals, hydration, and nutritional needs
• Maintain a clean, safe, and comfortable environment for patients and residents
• Document care activities accurately in accordance with facility policies and compliance standards
• Communicate effectively with patients, families, and interdisciplinary care teams

Requirements:

• Active Vermont Licensed Nursing Assistant (LNA) certification in good standing
