How you'll make an impact in this role:
  • Assess participants’ social, emotional, economic, and environmental needs, and develop appropriate intervention plans; home visits may be required.
  • Conduct interviews, prioritize needs, and accurately document case activities and progress.
  • Collaborate as a team-oriented department with managers, other departments, agencies, and caregivers to ensure alignment with treatment plans and continuity of care.
  • Connect participants and caregivers with community resources, supporting discharge planning and ongoing post-care needs.
What minimum qualifications you'll need:

Licensure / Certification / Registration:

 

Preferred Credential(s):

  • Drivers License.
  • Masters Level Social Worker.

Education:

  • Master's degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.
