We’re seeking a Licensed Master Social Worker (LMSW) to support General Inpatient (GIP) hospice patients across Dallas and Collin County—providing critical emotional, psychosocial, and crisis support to patients and families during some of life’s most vulnerable transitions.

Core Responsibilities

As our GIP-focused LMSW, you will:

  • Provide intensive psychosocial support to patients receiving General Inpatient hospice care
  • Lead crisis intervention efforts, helping families navigate urgent and emotional situations with clarity and compassion
  • Facilitate family meetings and mediation, ensuring patient-centered, goal-aligned care
  • Partner closely with hospital case managers, nursing teams, and interdisciplinary staff
  • Support discharge planning and care transitions, ensuring continuity and dignity in care
  • Deliver education and emotional support to caregivers and loved ones
  • Maintain timely, accurate documentation in alignment with hospice regulations

  Requirements & Qualifications:

  • Education: Master’s Degree in Social Work (MSW) from a state-accredited program
  • Licensure: Current Texas Certification (LMSW)
  • Experience: At least one (1) year of social work experience in a health care agency.
  • Skills: Strong interpersonal skills and a deep understanding of community resources.
  • Compliance: Must pass criminal background checks and provide proof of TB testing and a Hepatitis profile/acceptance.
  • Transportation: A valid Texas Driver’s License, reliable transportation, and valid auto liability insurance for local travel to patient homes.

Working Conditions

This position involves traveling locally to visit patients in their homes. You should be comfortable working in various conditions, including occasional exposure to infectious diseases and unpleasant weather. The role requires prolonged standing, walking, and the ability to make quick decisions under stressful conditions to meet patient needs.
