JOB TITLE: Licensed Clinical Social Worker (LCSW)

REPORTING TO: Mental Health Program Development Manager

JOB SUMMARY: The Licensed Clinical Social Worker (LCSW) at the Health Center is a key member of our integrated care team, responsible for delivering high -quality, patient -centered mental health services. This role involves providing evidence -based psychotherapy, crisis intervention, and collaborating with primary care providers, case coordinators, and community health workers.

Duties and Responsibilities:

Clinical Duties:
<\/p>

  • Deliver evidence -based psychotherapy to individuals, couples, and families.
    <\/li>
  • Collaborate with primary care providers to conduct warm handoffs for timely mental health interventions.
    <\/li>
  • Perform crisis interventions and create safety plans as needed.
    <\/li>
  • Maintain accurate, timely clinical documentation in the Electronic Medical Record (EMR) system, adhering to FQHC and state regulations.
    <\/li>
  • Uphold patient confidentiality and comply with HIPAA regulations and quality management policies.
    <\/li>
  • Maintains all BLS certifications and state licensure up to date
    <\/li><\/ul>

    Administrative Duties:
    <\/p>

    • Assist with the completion of intake forms and other clinical documentation.
      <\/li>
    • Participate in multidisciplinary team meetings to develop and update integrated care plans.
      <\/li>
    • Participate in quality improvement activities to align mental health services with the center’s mission and values.
      <\/li>
    • Ensure all clinical services meet the Health Resources and Services Administration (HRSA) Health Center Program standards.
      <\/li>
    • Stay informed about and comply with all state of Nevada FQHC requirements, including billing, reporting, and documentation standards.

      • The LCSW ensures compliance with state and FQHC regulations and adapts to evolving program requirements, supporting our mission to offer holistic care to underserved populations.

      • The LCSW ensures compliance with all 19 Health Resources and Services Administration (HRSA) Health Center Program Requirements.
        <\/li>
      • The LCSW is also expected to actively participate in and support quality improvement activities and the clinical practice team efforts emphasizing the importance of the individual patient and putting the patient’s needs first.

        Requirements<\/h3>

        Qualifications:
        <\/p>

        • Master's degree in social work from an accredited institution - required
          <\/li>
        • Active LCSW license in Nevada - required
          <\/li>
        • At least 2 years of clinical experience with a focus on psychotherapy - preferred
          <\/li>
        • Experience working in an integrated care setting, preferably within a primary care environment – preferred
          <\/li>
        • Familiarity with FQHC regulations and requirements – preferred
          <\/li>
        • Fluency in Spanish and English - preferred
          <\/li><\/ul>

            TYPICAL PHYSICAL DEMANDS:
            <\/p>

            Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, pulling and lifting and/or moving up to 25 pounds. Frequent sitting. Manual dexterity. Good reading eyesight and vision for close work. Ability to communicate by speaking and hearing in person or on the phone.
