Spectrum Healthcare Resources has an opportunity for a civilian LCSW -Special Needs Coordinator (SNC) at Minot Air Force Base. 

 

This Social Worker - Special Needs Coordinator contract position offers:

• Outpatient Clinic   
• Monday through Friday hours
• No night, No weekends, No On-Call
• 11 Paid Holidays

Job Requirements:

The LCSW / Special Needs Coordinator (SNC) position will have the following requirements:

 

• Sufficient knowledge to effectively apply Special Needs Coordination functions (Assessment, Planning, Facilitation & Advocacy)

• A minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years.

•  Must have  a Master of Social Work (M.S.W.) degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE).

• LCSW License from any state.
