Committed to Our Community in the Heart of the Redwoods. Removing Barriers to Healthcare Access
PCHC Plaza Community Health Center
Join a Team That Cares for the Whole Person-and the Whole Community
At Open Door Community Health Centers, we believe every patient deserves compassionate, whole-person care that supports their physical, emotional, and social well-being. We're looking for a Behavioral Health Clinician (BHC) to join our integrated care team at Plaza Community Health Center, Arcata, California, the heart of the true north coast.
The Opportunity
As a BHC, you'll play a key role on our interdisciplinary care team-collaborating with physicians, nurse practitioners, case managers, and dental/medical assistants to identify, triage, plan, manage, and provide primary behavioral health services for patients with overlapping physical and behavioral health needs. You will provide assessment, therapeutic treatment, referral, support, education, and skill-training through a variety of strategies focused on addressing specific behavioral changes in support of improved physical, social, and mental health. Patients are served in individual settings, in-person or via tele-health.
This is a dynamic position ideal for someone who values collaboration, continuity of care, and community impact.
