    Description

    Under the direction of the CEO, the Licensed Clinical Social Worker (LCSW) will provide direct services tosenior adults and caregivers from a variety of referral sources.
    This position connects patients andcommunity members to program offerings and community resources and may also serve as arepresentative.
    The LCSW will also work in conjunction with the patient’s family, caregiver, nurse, andclinician to provide optimal support services.
    This position will help the patient gain the strength tocarry on with daily like skills, improve quality of life and help them remain independent.

    90-Day Incentive Bonus ranging from $300 to $600- & first-year incentive for maintaining volumes
    Both In-Person and Telehealth visits with clients – must be willing to conduct some visits
    in-person.
    Territory of your choice in Illinois or Nevada.
