Job Title: Licensed Clinical Social Worker (LCSW) Location: Hyannis, MA 02601 Job Type: Contract (26 Weeks, with possibility of extension)
Schedule:
Shift: Day Shift (8:00 AM – 4:00 PM)
Hours: 40 hours per week
Rotating days and weekends
Job Summary:
We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to provide comprehensive social work services in a healthcare setting. The ideal candidate will support patients and families through psychosocial assessments, care coordination, discharge planning, and connection to community resources.
Responsibilities:
Conduct psychosocial and risk assessments, including mental health, substance use, family, and environmental factors
Develop and implement discharge and aftercare plans in collaboration with the care team
Provide counseling, crisis intervention, and emotional support to patients and families
Coordinate with healthcare providers and community organizations to ensure continuity of care
Assist patients with access to services, medical equipment, and long-term care placement
Maintain accurate and timely documentation in accordance with policies and ethical standards
Requirements:
Master’s degree in Social Work (MSW)
Active LCSW or LICSW license required
Strong communication, organizational, and problem-solving skills
Ability to work effectively in a fast-paced, high-pressure environment
Basic computer proficiency
CPI/MAB certification (or willingness to obtain)
Preferred Qualifications:
Prior hospital or healthcare setting experience
