    We are seeking an LAC to work in our Community Health Center. We provide outpatient primary care and behavioral health services. Job Title: Licensed Addiction Counselor (LAC)

    Summary: This position has been developed to integrate Substance Use Services into the Primary Care Setting. The LAC will assist in the development and implementation of Substance Use Services into the clinical setting and provide care coordination and a broad array of supportive and direct services to Sapphire Community Health patients.

    Essential Duties and Responsibilities:

    Other duties may be assigned.

    • Works closely with medical providers in identifying, referring and integrating behavioral health services.
    • Conducts Substance Use Assessments.
    • Develops integrated treatment plans and coordinates care with Primary Care and other providers.
    • Serves as resource to, and consults with, agency providers on issues related to Substance Use as needed.
    • Provides care coordination and compliance oversite for individuals receiving Medication Assisted Treatment (MAT) or receiving mandated treatment.
    • Provides individual, group, and family treatment and recovery support in accordance with evidence-based best practice.
    • Maintains accurate and timely clinical documentation.
    • Prepares and maintains reports and case histories in accordance with best practice and program requirements.
    • Participates in meetings and training as required

    Education and/or Experience:

    • Licensed Addiction Counselor (LAC) in the State of Montana
    • Minimum of 2-years' experience conducting assessments and treatment in a clinical setting preferred.
