PURPOSE: Planning and managing patient care according to each patient's needs. Interviewing patients and recording their medical history. Obtaining patient vital signs and escorting patients to their rooms. Provides general nursing care to patients in ACH facility by performing the following duties.

Salary : $32.00 (starting Salary)

Duties and Responsibilities include the following:

  1. Follows all PCMH procedure and policies in a culturally sensitive manner.

  2. Explains procedures and treatments to patient to gain cooperation, understanding, and allay apprehension. Reviews normal lab and diagnostic results with patients.

  3. Instructs patients regarding laboratory diagnostic procedures and assist with laboratory test.

  4. Administers prescribed medications and treatments in accordance with approved nursing techniques with proper documentation as well as administers appropriate medication and appropriate treatment plans according triage protocol.

  5. Maintains awareness of comfort and safety needs of patient.

  6. Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents.

  7. Follow triage protocol including telephone calls, chief complaint and walk-ins

  8. Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of patient as well as supervise medical assistants and is available to assist medical assistants.

  9. Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.

  10. Initiates a patient education plan according to the individualized needs of the patient, as prescribed by Physician and/or hospital policy including patient and family instruction.

  11. Rotates among various clinical services of institution such as obstetrics, surgery, orthopedics, outpatient and admitting, pediatrics, and psychiatry.

  12. Prepares rooms, sterile instruments, equipment, and supplies for surgery, keeping departmental inventory of vaccinations and supplies.

  13. Responsible for maintaining emergency kit, oxygen tank, spill kit, refrigeration, medical equipment, etc. with monthly inspection and documentation.

  14. Helps patients with patient assistance programs or other pre-authorization with pharmaceutical companies in obtaining reduced cost or free medication.

  15. Maintain HIPPA compliance

  16. Dispose of hazardous material using HAZMAT protocols.

  17. Performs secretarial tasks such as answering the phone, taking messages in an accurate efficient manner, communicates with pharmacies.

  18. Participates in department or unit quality improvement and risk management activities and is required to attend all meetings.

  19. All other duties as assigned.

Requirements:

  • Computer Skills - Must be computer savvy; having a general knowledge of Microsoft Office. The Licensed Practical Nurse will be required to undergo ECW training.

  • Education and/or Experience: Associates Degree required or Completion of an accredited LPN or LVN (Licensed Vocational Nurse) program.

  • Experience: minimum1 year of experience in out patient setting

  • Certificates, Licenses, Registrations: Must maintained an active New Jersey LPN license and maintain current education credits. Have basic CPR training for Adult and Pediatrics. ACLS, PALS preferred.
